Personal Licence Forms

If you wish to apply for a Personal Licence to authorise the sale of alcohol you will need to complete the following forms. The Personal Licence Guidance will assist you in making your application.

The Guidance refers to one of the following criminal record checks being included within the application.

  • a criminal conviction certificate issued under section 112 of the Police Act 1997,
  • a criminal record certificate issued under section 113A of the Police Act 1997, or
  • the results of a subject access search under the Data Protection Act 1998 of the Police National Computer by the National Identification Service.

The Criminal Conviction Certificate is the ‘Basic’ Criminal Record Bureau (CRB) check but it is not yet available in England and Wales but is available through Disclosure Scotland information at the link below.

The Criminal Record Certificate is the ‘Standard’ CRB check. This is only available through registered bodies and the Criminal Record Bureau are not currently processing applications for this type of disclosure.

The subject access search is available through Lincolnshire Police Disclosure Unit, on 01522 558247.
